I suggest we use this thread to recount close calls and mistakes, things that didn't necessarily result in injury, but could have.

I think we can learn from each others mistakes, even if they didn't result in injury.

Example: In my last trip down Baileys canyon, we used a thick downed log as an anchor, only to realize after several of us had rapped down, that it was rotten thru and thru, and really wasn't suitable as an anchor.

On the same trip one person did not bring a wet suit, following the advice of an experienced (and obviously warmer blooded companion) and got really cold, and had difficulty as a result, and had to be lowered down a number of raps.

Knowing your own limitations for cold is something that only comes with experience. If you are giving advice on thermal needs it's best to error on the side of caution.
